Case Manager P&I
Job Purpose:
- The Case Manager - Inland P&I at Lutheran Social Services of Northern California is responsible for providing comprehensive case management services to clients in need. This role is pivotal in supporting individuals and families by assessing their needs, developing personalized service plans, and coordinating with various service providers to ensure the delivery of essential resources. The Case Manager will work closely with clients to empower them towards achieving self-sufficiency and improving their overall quality of life.
Key Responsibilities:
- Conduct thorough assessments of clients' needs and develop individualized service plans tailored to their specific circumstances.
- Provide ongoing case management support, including regular follow-ups to monitor progress and make necessary adjustments to service plans.
- Coordinate with internal and external service providers to ensure clients have access to necessary resources such as housing, healthcare, employment, and educational opportunities.
- Maintain accurate and up-to-date case files and documentation in compliance with organizational and legal standards.
- Advocate on behalf of clients to secure services and resources that will aid in their personal development and stability.
- Facilitate group sessions and workshops aimed at enhancing clients' skills and knowledge in areas such as financial literacy, job readiness, and health management.
- Collaborate with other team members and stakeholders to develop and implement strategies for improving service delivery and client outcomes.
- Stay informed about community resources, trends, and best practices in case management to continually enhance the quality of services provided.
- Participate in training and professional development opportunities to maintain a high level of expertise in case management practices.
Qualifications
Required Education:
- Bachelor’s degree in Social Work, Psychology, Sociology, or a related field. A Master’s degree is preferred.
Required Experience:
- Minimum of 3 years of experience in case management or a related field.
- Proven experience working with diverse populations, including individuals with mental health challenges, substance abuse issues, or those experiencing homelessness.
- Experience in developing and implementing individualized care plans.
- Previous experience in a non-profit or social services environment is highly desirable.
Required Skills and Abilities:
- Strong understanding of case management principles, healthcare, and social service systems.
- Excellent communication skills, both verbal and written, with the ability to interact effectively with clients, colleagues, and external partners.
- Demonstrated ability to manage a caseload efficiently and prioritize tasks in a fast-paced environment.
- Proficient in using case management software and Microsoft Office Suite.
- Ability to work independently as well as collaboratively within a team setting.
- Strong problem-solving skills and the ability to make sound decisions under pressure.
- Sensitivity to cultural and socioeconomic diversity and the ability to work with individuals from various backgrounds.
- Valid driver’s license and reliable transportation for travel within the service area.
Salary : $25 - $27
