- Assist the Director of Clinical Services in completing initial, quarterly and annual resident assessments, and all other required service plans and assessments.
- Assist the Director of Clinical Services in scheduling staff, maintaining staff coverage, and supervising supportive living staff.
- Follow all standards regarding safety, licensing, regulatory and agency requirements.
- Responsible for maintaining resident files in accordance with HFS requirements, completing resident care plans.
- Assist the Director of Clinical Services in medication set up and dispensing; communicate with pharmacy regarding resident medication and concerns.
- Work in conjunction with the Dir. of Clinical Services to perform on-call service as scheduled; respond promptly to emergencies; and provide shift coverage as needed.
- Work with Food Service Manager to provide input for meal plans
- Work with Activity Coordinator to plan and schedule appropriate activities for residents.
- Performs other related tasks as needed.
- High school diploma or equivalent preferred; at least 18 years of age.
- Complete a Two-Step Tuberculin Test.
- Pass Health Care Worker Background Check.
- Be willing to participate in required training as necessary to meet requirements of the Supportive Living program.
- Background check clearance and Health Care Worker background check clearance required.
- Completion of a two-step Tuberculin test required.
- LPN, licensed by the State of Illinois, required.
- Certified in emergency resuscitation.
- Must be at least 18 years of age or older.
- Minimum of three (3) years’ experience in healthcare services to adults with disabilities or the elderly population in assisted living, inpatient hospital, long-term care setting, adult day care, or in a department approved health related field.
- Minimum of one (1) year experience in a supervisory/managerial role required.
- Demonstrated ability to communicate in a clear, comprehensible manner, verbally and written.
- Excellent communication, organization, presentation and pc/computer skills (including working experience with Microsoft Office – Outlook, Word, Excel and PowerPoint) along with other related software.
