Position Summary:
- The Permanency Support Specialist works with and provides ongoing support services to children who have achieved permanency of adoption/guardianship, and advocates and provides supports for continued permanency and stability for that child.
- Provide supportive case management services to eligible children and families in the Post Adoption/Guardianship Program.
- Collaborate with Wendy’s Wonderful Kids (WWK) staff to complete transition of families following successful guardianship or adoptive placements.
- Utilize assessment tools and collect outcomes to focus services on more effective approaches for adoptive families.
- Respond to crisis, correspondence, telephone calls and other inquiries from adoptive/guardianship family members in a prompt, caring and understanding manner.
- Meet with the family as appropriate to the family’s needs and contract requirements and maintain phone and e-mail contact on an ongoing basis.
- Support adoptive/guardianship families in addressing behavioral or other problems within the home through phone contact, face-to-face visits and referral/resource information.
- Advocate for the family in seeing that all physical and emotional needs are being met.
- Provide regular follow up to families who have experienced successful guardianship or adoptive placements for eligible children and families.
- Performs other duties as assigned.

- Bachelor’s degree in social work, human services, or related field required.
- At least one year of experience working with children and families preferred. Customer service experience required.
- Knowledge of case management, child welfare system, and adoption related issues to include attachment and grief.
- Demonstrated understanding of and ability to work with people of diverse backgrounds.
- Valid driver's license, current auto insurance, and ability to drive for organization business.
- Prolonged periods of sitting at a desk and working on a computer.
- Hybrid work environment (field, in-office, and remote).
- Company-issued laptop and cell phone.
- Ability to drive and transport clients within the needed service area.
- Occasional overnight travel (as needed).
- Ability to work flexible schedule including evenings and weekends to meet client needs (as needed).
