LTC Therapy is seeking a Physical Therapist (PT) to join our rehabilitation team in Harwich, MA. This full-time opportunity is ideal for a clinician who enjoys helping residents restore mobility, improve strength, and achieve meaningful functional goals in a skilled nursing and post-acute care environment.
What You'll Do
- Evaluate residents and develop individualized physical therapy plans of care
- Provide skilled treatment focused on mobility, gait, balance, strength, transfers, and endurance
- Establish functional goals and monitor progress throughout the rehabilitation process
- Modify treatment plans based on resident needs and response to therapy
- Educate residents and caregivers on exercises, mobility strategies, and safety techniques
- Complete accurate and timely clinical documentation
- Collaborate with PTAs, OT, SLP, nursing, and other members of the care team

Qualifications
- Active Massachusetts Physical Therapist license required
- Graduate of an accredited Physical Therapy program
- Strong clinical, communication, and teamwork skills
- Skilled nursing or rehabilitation experience preferred, but not required
