Day ShiftAbout the Role:
We are seeking a dedicated and compassionate Part-time Social Worker to join our team at Loyola Medicine. As a social worker at MacNeal Hospital, you will have the opportunity to make a meaningful impact in the lives of our patients and their families.
Responsibilities:
Support patients and families by providing biopsychosocial assessments, counseling, and case management services.
Collaborate with healthcare providers to identify and address mental health needs, including depression, anxiety, and trauma.
Develop and implement care plans to promote patient independence, self-advocacy, and community integration.
Provide education and support on adjustment to chronic disease, life planning, and end-of-life issues.
Counsel patients on entitlements, medications, transportation, and advance directives.
Identify and refer patients to specialized services, such as drug and alcohol treatment, legal services, financial and employment counseling, and housing support.
Requirements:
Master's degree in social work (MSW) from an accredited program required.
Licensed Clinical Social Worker (LCSW) certification in Illinois required.
Minimum of six months of prior social work experience in a hospital setting preferred.
Familiarity with Joint Commission standards and crisis intervention techniques essential.
