Key Responsibilities:
- Support referrals from providers in the ambulatory setting, seeking biopsychosocial and behavioral health services for patients.
- Identify, assess, and manage mental health conditions, such as depression and anxiety.
- Provide case management and care coordination for individuals with chronic and complex medical conditions.
- Patient navigation, particularly for those moving among different healthcare levels (e.g., inpatient, outpatient, home health, or long-term care).
- Offer counseling on adjustment to chronic disease, life planning, and end-of-life issues.
- Refer patients for specialized services, such as drug and alcohol treatment, legal services, financial and employment counseling, and housing support.
Requirements:
- Master's degree in social work and LCSW licensure in Illinois required.
- Minimum of six months of prior social work experience in a hospital setting.
- Familiarity with Joint Commission standards.
- Able to manage crisis situations calmly and effectively.
- Able to work under stressful conditions and in difficult situations.
