Salary: $90,000 - $98,000/ Annually
What You’ll Do:
- Perform resident skin assessments, obtain treatment orders, administer treatment as ordered, and document appropriately to ensure that resident skin integrity is improved and/or maintained.
- Documents findings appropriately and contacts physician to make recommendations for new or revised treatment orders; Monitors the progress of wound healing and makes recommendations for changes as appropriate.

- Maintain records of pressure ulcers and skin alterations including risk assessment, prevention and minimizing activities; Performs and records weekly pressure ulcer assessment and measurement documentation; Identify and maintain effective methods to gather pressure ulcer and skin alteration related information; Calculates weekly / monthly pressure ulcer rates.
- Investigate and identify center practices and risk minimizing measures, including accurate completion of Braden risk scales on admission, and implementation of preventive interventions.
- Monitor timely implementation of preventive interventions for at risk residents, including new admissions, individuals with skin alterations and individuals at risk for development of skin alterations.
- Educate newly hired nursing staff on Skin Practice Guide requirements and Skin Resource Library including wound assessments, preventive interventions, care planning, treatment selection options, documentation requirements and education resources available.
- Evaluate availability of needed supplies for wound care and coordinate ordering with central supply.
- Assist the Director of Nursing and medical director with case review for center acquired pressure ulcers.
- Monitor changes in state and federal regulations and current industry-standard practices; Keeps current with industry trends through ongoing specialized education in the area of skin and wound management
- Provide monthly, quarterly and annual reports including recommendations for changes in center practice for the Quality Assurance and Performance Improvement Committee.
- Other duties, responsibilities and activities may change or assigned at any time with or without notice.
- Current LPN/RN license for the State of Colorado
- Current BLS/CPR certification or willing to obtain upon hire.
- Experience as a wound/treatment nurse
- One-year experience providing specialized treatments in long-term care setting
- Minimum 2 years of nursing experience in a Skilled Nursing Facility
- Wound Care Certification (preferred)
