JOB TITLE: Lower Sioux Wokheya Case Manager
DEPARTMENT: Lower Sioux Indian Housing Authority
REPORTS TO: Housing Director
FLSA STATUS:
STATUS: Part Time – Occasional Evening/Weekend Hours
SALARY GRADE:
SUMMARY:
This is a new grant funded position through the MN Department of Human Services grant, that will work with homeless or precariously housed community members. The person in this position will provide case management to community members in finding stable housing, job applications and other ongoing services if needed.
EDUCATION AND EXPERIENCE:
Experience with resource navigation.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provide the following essential components of Housing Support case management to Lower Sioux Wokheya residents to help them move into housing and work with them to maintain housing:
- Assessment: Work with participants to assess strengths and identify barriers to permanent housing.
- Plan development: Develop specific, individualized service plans based on the assessments.
- Coordination of services: Connect participants to necessary services; coordinate services and work with other service providers to ensure access, eligibility and follow through by providers and participants.
- Monitoring: Conduct an ongoing evaluation of the participants’ progress through the plan, reassessment, and adjustments to the plan.
- Provide intake services to applicants, understand and maintain resident/client confidentially and data privacy standards.
- Create client files including all relevant assessments, plan and evaluation information and update regularly related to Housing Support.
- Help with regular reporting as required by the grant.
- Work with landlords in the community to help them understand and be willing to use the Housing Support program to pay for rent.
- Participate in staff meetings, in-service training, and other relevant training.
- Refer non-standard situations to Executive Director for final decision.
- Keep updated resource information on local and regional human services and make these available for all shelter staff and residents.
- Establish and maintain positive networking relationships with other Tribal Program staff and area human service agency staff to facilitate resident/client referrals.
- Attend community meetings as requested by Executive Director.
- Regular attendance necessary.
- Regular punctuality necessary.
- Available and willing to work any changes in hours on the schedule at any time during employment.
- Other duties as assigned
SKILLS KNOWLEDGE AND ATTRIBUTES:
- Understanding of public assistance programs
- Effective verbal and written communication skills, basic computer skills.
- Knowledge of and experience working with Native American People
