CHARACTERISTICS OF THE WORK
Under general direction, works with medical providers, primary care teams, and social service agencies to provide short-term care coordination in both clinical and community-based settings, including patients’ homes.
Responsible for assisting patients with navigating the healthcare system and troubleshooting barriers to patient care.
Serves as a cultural liaison, using cultural and health science knowledge and skills to negotiate with patients and the healthcare system for an effective, beneficial healthcare plan.
Examples of Duties

Collaborates with a healthcare team, referral staff, and community partners to identify those in need of care coordination services.
Connects patients with community support services.
Identifies gaps in healthcare services and develops program plans to meet those needs.
Develops, implements, and evaluates individual patient care coordination plans.
Documents patient/worker activity for care coordination plans.
Supports patients in culturally appropriate ways to promote their health and manage their chronic illnesses.
Contacts patients to inform them of or remind them about appointments and/or transportation plans.
Monitors the delivery and outcome of services provided to patients by medical facilities, social service agencies, and community resources.
Offers cross-cultural education and trainings for Family Health Centers (FHC) and community partners.
Provides medical language interpretation and translation services for patients and for FHC as needed.
Stays informed of changes to health insurance and the healthcare system.
Completes documentation required by funding sources and participates in program evaluation.
Participates in staff, community, and statewide workgroup meetings.
Performs related work as required.
Minimum Requirements
MINIMUM REQUIREMENTS
High school graduation or its equivalent.
Fluent in Spanish and English languages.
One year of work experience participating in community outreach, conducting health promotion activities, or assisting patients with navigating the healthcare system, in a community health or social service setting.
Must successfully complete the Kentucky Community Health Worker training and certification within one year of employment and maintain certification throughout employment with FHC.
An equivalent combination of training and experience may be substituted as determined applicable by Civil Service.
Physical Requirements
Responds to patient and FHC needs via telephone, internet, fax, computer software, and in person.
Spends extended periods of time sitting, standing, walking, climbing, bending, and reaching.
Spends extended periods of time reading computer screens.
Lifts and moves up to 10 lbs. with or without assistance.
Special Requirements
Must provide own transportation to work at other locations as needed.
