Responsibilities
- Build and maintain strong referral relationships with hospitals, discharge planners, and community partners to support census goals.
- Maintain an updated and accurate nursing home census and track bed availability.
- Meet and support census development and annual budget expectations.
- Respond promptly to online referral and discharge planning systems.
- Interview prospective residents and families; conduct professional facility tours.
- Present and review all admission agreements, facility policies, and financial expectations.
- Coordinate all medical documentation, verify insurance and financial eligibility, and ensure a smooth admissions process.
- Collaborate closely with Nursing, Business Office, and Social Work teams to ensure timely and coordinated admissions.
- Complete all administrative and admissions paperwork accurately and on schedule.
✅ Requirements
- High school diploma required; Bachelor’s degree in Healthcare Administration, Business, Marketing, Social Work, or related field preferred.
- 1–3 years of experience in long-term care, skilled nursing, or healthcare admissions, marketing, or case management strongly preferred.
- Experience working with hospitals, referral sources, or discharge planners is a plus.
- Knowledge of Medicare, Medicaid, Managed Care, and private insurance eligibility preferred.
- Strong ability to multitask, prioritize, and maintain accuracy in a fast-paced SNF environment.
- Proficiency with EHR systems, referral software, and Microsoft Office.
- Manager on Duty Rotation
