Individual Support Planning
1. Knows the content of each Individual Support Plan (ISP) and partners with persons supported to follow through on goals.
2. Supports people in activities of their own choosing at home and in the community; helps people discover new areas of interest.
3. Assists individuals to develop a positive self-image; encourage independence with adult day living skills providing assistance whenever needed.
Documentation and Follow- Through
- Reads the house communication log at the beginning of each shift
- Writes a clear, brief summary of the shift prior to the end of each shift
- Completes all required daily documentation as related to the ISP.
- Is accountable for and properly documents the use of money.
Health/Safety/Wellbeing
- Knows and follows procedures for specific health care plans and health issues.
- Ensures medications are administered according to the Looking Upwards Medication Administration Policy.
- Operates all vehicles defensively, utilizing seatbelts and obeying all traffic laws.
- Reports any household, vehicle, maintenance or other safety issues immediately.
- Knows the whereabouts of persons supported at all times.
- Works with team members in keeping the home and vehicles clean at all times.
- Follows program plans and implements behavior management programs for each person supported.
- Informs the Director/Service Coordinator of any concerns regarding significant changes in an individual’s behavior.
- Follows dietary/nutrition guidelines.
- Utilizes the on-call system as specified by the program guidelines.
- Adheres to all agency policies regarding exposure to blood borne pathogens, safety, disaster plans, etc.
- Completes all behavior management training.
- Completes annual fire safety training.
- Completes all health and medical trainings.
- Completes all Human Rights trainings.
- Schedules and follows through on medical appointments as necessary.
- Adheres to all agency policies and procedures.
- Performs other duties as assigned.
- Provides holiday coverage and participates in filling open shifts.
Essential Qualifications and Requirements:
- Be at least 18 years of age with a high school diploma or GED
- Employees that are responsible for the transportation of individuals supported must have a valid driver’s license with acceptable driving record. DSPs may be required to use their personal vehicle with proof of valid insurance, registration and inspection.
- Ability to complete CPR, Safety Skills, Online Trainings and other required agency trainings
- Employees that are responsible for the administration of medications to individuals supported must complete and carry out Medication Administration
- Capable of effective oral and written communication
- Capable of reading and comprehending policies and instruction, and use varying degrees of independent judgment and discretion
- Ability to respond to emergency situations at all times
Essential Physical Demands:
Must be able to operate tools and equipment safely (agency van, phone, copy machine, overhead projector, computer and email). Must be able to exert up to 100 lbs with two people and lift up to 50 lbs independently. Physical demands are in excess of sedentary work as position frequently requires walking, driving, standing, sitting, reaching with hands/arms, kneeling, bending, twisting, stooping, pushing and pulling. Ability to carry out safety techniques and holds.
Work Environment:
Participation indoors as well as outdoors during all seasons
