What are the responsibilities and job description for the Social Services Assistant position at Longwood Health and Rehabilitation Center?
As the Social Services Assistant (SSA) you will assist the Director of Social Services in the ongoing efforts to facilitate and improve each resident’s social, emotional and psychological well-being. The ideal Social Services Assistant will be able to find new ways to add value daily, by easing the workload of the department and also the Director of Social Services.
```Duties```
- Assist in collecting data for the development of discharge plans,
- Collect data as it relates to the MDS and Care Plans for new admits
- Assist residents and families with adjustment to changes, including room changes.
- Document progress notes related to care plans
- Provide support and assistance to social workers in the delivery of social services
- Assist with the coordination and implementation of programs and services
- Conduct assessments and interviews to determine client needs and eligibility for services
- Maintain accurate and up-to-date client records and documentation
- Collaborate with other agencies and organizations to ensure comprehensive care for clients
- Participate in case conferences, team meetings, and trainings as required
```Qualifications```
- Participate in case conferences, team meetings, and trainings as required
- Minimum (1) year experience in social work, preferably in an LTC, SNF, or hospital facility.
- Ability to communicate at all levels of organization
- Polished Communicator, Customer Service Oriented
- Ability to work with minimal supervision, take initiative and make independent decisions.
- Approachable, flexible and adaptable to change
