Job description:
We’re looking for a caring and dependable Bilingual Licensed Practical Nurse (LPN) who speaks both English and Spanish to join our team for home visits in our GAFC/AFC programs. In this role, you’ll provide quality, compassionate care to a diverse group of patients, ensuring effective communication and personalized care. It’s a great opportunity to use your clinical skills while working closely with patients in a supportive, team-focused environment.
Responsibilities:
- Conduct monthly home visits to ensure services are being provided appropriately and in alignment with each member’s care plan
- Monitor members’ overall health, including observing changes in condition, and reporting any concerns promptly
- Provide guidance and support to caregivers, answering questions and reinforcing proper care techniques
- Communicate effectively with members, families, caregivers, and the care team to ensure coordinated care
- Maintain clear, accurate, and timely documentation in compliance with program and regulatory requirements
- Promote a safe, respectful, and supportive environment for all members
Requirements:
- Active Massachusetts LPN license in good standing
- Fluent in both English and Spanish (required) with strong verbal and written communication skills
- Reliable transportation and valid driver’s license to complete scheduled home visits
- Ability to travel consistently within assigned service areas
- Strong organizational skills and attention to detail for accurate documentation
- Comfortable working independently in a home care setting
- Basic computer skills for documentation and communication
- Prior experience in home care is a plus.
Additional information:
- $78,000/year
- Up to $200 monthly bonus
- Mileage reimbursement based on MA rate
- Sick time (40hrs per year)
- Vacation time (40hrs per year to start after 1 year of employment)

Salary : $200 - $78,000
