Position Overview
The Hospice Registered Nurse (RN) Case Manager is responsible for the overall management of a defined patient caseload, overseeing all clinical, regulatory, and coordination aspects of the patient's plan of care. This role sits at the intersection of clinical excellence, autonomy, and continuous service delivery. The Case Manager is accountable for high-quality clinical outcomes, timely and compliant documentation, and seamless coordination within the interdisciplinary team (IDT). This role requires strong clinical judgment, emotional intelligence, urgency, and professionalism. Case Managers are expected to operate with autonomy, accountability, and a deep sense of ownership over patient outcomes.
Key Responsibilities
Patient Care & Caseload Management
- Manage a defined caseload of mixed-acuity hospice patients.
- Conduct comprehensive initial and ongoing clinical assessments to determine hospice eligibility in accordance with Medicare hospice regulations and organizational standards.
- Develop, implement, and monitor an individualized, evidence-based care plan in collaboration with the patient, family, and IDT members.
- Provide expert symptom management, pain control, and crisis intervention as necessary.
- Deliver compassionate, high-quality end-of-life care that aligns with patient goals.
- Support hospital-based care coordination for GIP patients when assigned
Coordination, Compliance & Quality
- Ensure all documentation meets Medicare, state, and organizational compliance standards, maintaining an audit compliance rate of ≥ 99%.
- Complete all required consents, physician orders, and documentation accurately and in a timely manner.
- Coordinate the delivery of services, equipment, and supplies with intake, operations, and on-call teams.
- Participate in the Interdisciplinary Team (IDT) meetings to confirm prognosis, review care plans, and ensure appropriate resource utilization.
- Adhere to all hospice Conditions of Participation (CoPs), organizational policies, and SOPs.
Team & Partner Collaboration
- Act as a trusted clinical partner and primary liaison for the patient, family, and other healthcare providers.
- Collaborate effectively with the hospice medical director, social workers, spiritual counselors, and aides.
- Represent Loma Linda University Hospice professionally within community and facility settings.
- Strengthen referral relationships through responsiveness, credibility, and consistent follow-through.
Performance Outcomes & Key Metrics
Clinical Quality & Compliance (40%)
- Accurate hospice eligibility determinations and proper level-of-care assignments.
- Audit and chart review compliance ≥ 99%.
- Zero material documentation deficiencies.
- Demonstrable mastery of complex pain and symptom management.
Patient & Family Experience (40%)
- High patient and family satisfaction with clinical care and responsiveness.
- Consistent demonstration of professionalism, empathy, and clear communication.
- Positive feedback from hospital and referral partners regarding care delivery.
Team & Operational Contribution (20%)
- Efficient management of assigned caseload and timely completion of visits.
- Effective collaboration with intake, operations, and clinical teams.
- Active participation in training and continuous improvement initiatives.
Required Qualifications
- Active, unrestricted Registered Nurse (RN) license in the applicable state.
- Minimum of 2 years of hospice, palliative care, home health, or acute care experience.
- Strong knowledge of hospice eligibility criteria and Medicare hospice regulations.
- Exceptional communication, interpersonal, and clinical judgment skills.
- Proficiency with EMRs and clinical documentation systems.
- Valid driver’s license and reliable transportation.
Preferred Qualifications
- Prior case management experience, especially in hospice or home health
- CHPN (Certified Hospice & Palliative Nurse) or willingness to obtain.
- Experience working in hospital-based or joint venture hospice models.
- Bilingual skills (market-dependent).
