What are the responsibilities and job description for the Registered Nurse Case Manager/Admissions position at Serenity Hospice?
The Hospice RN Case Manager/Admission is an experienced registered professional nurse who initiates and coordinates the hospice care plan, provides skilled and palliative care and coordinated the services provided to his/her patients. The RN works cooperatively with the attending physician, Hospice Medical Director, and other members of the interdisciplinary hospice team. The RN participated in patient care conferences and supervises the Home Health Aides assigned. The RN provides care, as described in this position description, to patients who have been diagnosed with a terminal illness. The RN regularly reviews and update\s the plan of care. Performs prescribed medical treatment, including pain management and symptom control, conducts assessments and evaluations, provides education and supportive care to patient and caregiver.

- Supervises: Licensed Vocational/Practical Nurses, Certified Home Health Aides, Clinical team
Pay Rate:
- $56-$60/hour (Negotiable; Depending on experience )
Experience:
- 1 year minimum of Hospice and/or Home Health setting
- 1 year minimum at a Skill Nursing Facility
- Autonomy experience on practice
