Your expertise will support patient assessments, medication administration, wound care, and vital signs monitoring while ensuring compliance with healthcare regulations and confidentiality policies.

Responsibilities
- Deliver comprehensive nursing care to our valued residents.
- Perform patient assessments by evaluating vital signs, medical histories, and physical examinations to inform treatment plans.
- Administer medications and injections accurately while adhering to safety protocols and HIPAA regulations.
- Manage IV infusions, catheterizations & wound dressings.
- Utilize EMR/EHR systems to document medical records, treatment notes, and discharge instructions efficiently.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans for residents with complex needs such as dementia or acute pain management.
- Conduct triage assessments for urgent cases to prioritize patient needs effectively.
- Support infection control practices and adhere to long-term care regulations to ensure a safe environment for all patients.
Experience
- Valid LPN license with current state licensure required; additional certifications such as BLS/CPR are essential.
- Demonstrated experience working with geriatric patients in skilled nursing facilities or assisted living environments preferred.
- Proficiency with EMR/EHR systems.
- Hands-on experience with IV infusion therapy, medication administration, specimen collection/processing, and sterile procedures.
- Background in managing patients with dementia or Alzheimer's disease, including memory care strategies and behavioral health considerations.
- Knowledge of anatomy, physiology, pharmacology principles, and medical terminology necessary for accurate patient assessment and documentation.
