Job description
We're seeking a compassionate and organized Care Management Assistant to support our care managers and clients through hands-on coordination, logistical support, and non-clinical advocacy. You will work closely with clients, families, and interdisciplinary teams to implement care planning, support day-to-day client needs, and uphold LM Services' commitment to dignity and quality of life.
Key Responsibilities
- Assist in the implementation of client care plans, services, and appointments
- Support clients in navigating medical systems, including appointment scheduling, reminders, and transportation
- Provide in-home check-ins and observe/report changes in condition to the care management team
- Facilitate medication reminders, and ensure adherence to routines
- Update Care Managers about needs and condition changes
- Support documentation and recordkeeping of care activities and observations
- Help coordinate non-medical services such as light housekeeping, order supplies, manage salon appointments and other social activities
- Provide companionship and emotional support to promote client well-being and reduce isolation
- Attend appointments (when needed) to provide advocacy and ensure continuity of care communication
- Assist care managers with administrative tasks, such as follow-ups, care notes, and client files
- Participate in ongoing training to support evolving client needs and enhance service delivery
Qualifications
- Experience in healthcare, social services, elder care, or a related field preferred
- Strong organizational skills and attention to detail
- Empathetic, patient, and respectful in client interactions
- Skilled communicator with the ability to interact effectively with clients, families, and professionals
- Ability to work independently and as part of a collaborative team
- Comfortable visiting clients in their homes; access to reliable transportation required
- Tech-savvy and able to use basic care management software and digital tools

Job Types: Part-time, Internship
Pay: From $30.00 per hour
Expected hours: 15 – 25 per week

Salary : $36,000 - $72,000
