This is a professional contract role for experienced care managers who want autonomy with structure, clarity, and respect.
Position OverviewThe Independent Contractor Care Manager provides Aging Life Care Management services to older adults and adults with complex medical or functional needs.
You will work within a thoughtful, well-defined care management framework while bringing your own clinical judgment, experience, and understanding to every client relationship. Established processes and documentation standards provide structure, while teamwork, collaboration, and professional discretion guide day-to-day care.
Scope of Work- Conduct comprehensive biopsychosocial assessments
- Develop, implement, and update individualized care plans
- Coordinate care across home, hospital, rehabilitation, and senior living settings
- Provide ongoing life care management and monitoring
- Navigate healthcare systems and collaborate with medical providers
- Advocate for clients and families with professionalism and empathy
- Educate and support family caregivers
- Complete timely, accurate documentation
- Help prevent inappropriate placements, duplication of services, and unnecessary hospitalizations
- Follow established policies, procedures, and escalation guidelines designed to support quality care
We welcome professionals with direct care management or senior care coordination experience, including but not limited to:
- Registered Nurses (RN)
- Licensed Practical Nurses (LPN)
- Licensed Social Workers (LSW, LCSW, MSW)
- Care Managers / Case Managers
- Gerontology-trained professionals
- Professionals with experience in home health, hospice, palliative care, rehabilitation, hospital discharge planning, or senior living
What matters most is experience, clinical judgment, professionalism, and a commitment to excellent care—not a single credential.
- Demonstrated experience in care management, case management, or care coordination
- Strong assessment and care-planning skills
- Clear, compassionate written and verbal communication
- Excellent organizational and time-management abilities
- Confidence navigating healthcare systems and community resources
- Ethical practice and strict respect for confidentiality
- Cultural sensitivity and a client-centered approach
- Comfort with technology (training provided)
- Ability to work as a 1099 independent contractor
- Active professional license or credential (RN, LPN, or applicable license)
- Professional liability insurance
- Successful completion of a background check
- Reliable transportation and ability to travel locally
- Use of own equipment while adhering to documentation and communication standards
