We are seeking a Nurse Practitioner or Physician Assistant to join our interventional pain management team across multiple locations, including outpatient clinics, a physical therapy office, and a surgery center.

Responsibilities include:
- Conduct comprehensive assessments, diagnoses, and care plans
- Order, interpret, and follow up on diagnostic studies
- Document patient care accurately
- Provide care per accepted standards and collaborate with team members
- Monitor and refer patients to specialists as needed
Qualifications:
- Current applicable state license and national certification
- DEA license or ability to obtain one
- Basic Life Support certification
This full-time role offers a salary between $125,000 and $150,000 annually. Candidates must be willing to relocate to Sutherlin, OR before starting. Work is in person, with a focus on outpatient care.
