Responsibilities include administering medications, wound care, assisting with activities of daily living, supervising nursing assistants, monitoring residents’ physical and emotional status, and developing personalized care plans in collaboration with the interdisciplinary team.
The role involves accurate clinical documentation, coordinating admissions, transfers, and discharges, and participating in quality assurance, staff education, and regulatory surveys.
Qualifications: Active LPN/LVN license, CPR/BLS certification preferred, minimum one year of clinical nursing experience (long-term care experience preferred), and familiarity with electronic health records and Microsoft Office.
