Job Summary: The Patient Care Coordinator is responsible for the coordination of patient care. The Patient Care Coordinator will function as a patient advocate, providing support during the treatment process.
Responsibilities and Duties:
- Supports and activates patients to enhance their engagement in treatment.
- Corresponds with referral sources, legal systems, and outside providers as needed.
- Ensures proper consent/release forms are signed by patients.
- Arranges patients’ aftercare and discharge planning as directed by the primary treatment team.
- Facilitates the completion of FMLA, COBRA, and Short-Term Disability paperwork.
- Assists the primary treatment team with patient transfers as requested.
- Schedules and confirms patient appointments for services including outpatient, mental health and MAT.
- Facilitates patient placement in recovery housing, long term facilities, and halfway housing.
- Gathers feedback on placement agencies to assess patient safety and continued use of resource.
- Attends daily treatment team meeting and provides input.
- Manages the release of patient records upon discharge, as requested.
- Facilities outreach to address occupational, legal, and familial issues.
- Supports data collection.
- Provides patients with a satisfaction survey upon discharge.
- Remains in communication with Admissions staff regarding patient financial obligations.
- Meets with patients within the first few days following admission to discuss financial obligations and follows up as necessary.
- Arranges payment and/or payment plan and completes the necessary documentation.
- Performs other related duties as assigned.
Education and Experience:
- High School diploma or equivalent.
- CRS certification preferred.
- Long-term recovery experience.
- Experience in an addiction treatment setting.
- Familiarity with related community resources.
