Join a dynamic home-based healthcare team as an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant).
Responsibilities include:
- Conduct comprehensive in-home health assessments, including medical history, physical exams, screenings, and point-of-care tests.
- Identify and document diagnoses for care management and collaborate with primary care providers.
- Recognize urgent situations and intervene appropriately.
- Educate patients on disease management, medications, and health compliance.
- Address social determinants of health and provide necessary referrals.
- Active, unrestricted licensure in the practicing state and relevant national certifications.
- Reliable transportation and a valid driver's license.
- Bilingual in Spanish preferred.
- At least 1 years of clinical experience, especially in family, geriatric, or home health care.
- Ability to navigate varied home environments and perform assessments in diverse settings.
- Proficiency with electronic medical records and technology.
- Strong communication skills with geriatric or Medicare populations.
- Comfortable transporting equipment and navigating stairs.
