LPN Medical Care Manager

Role:

Licensed Practical Nurse (LPN) Medication Nurse responsible for providing safe and accurate medication administration, health monitoring, and clinical documentation in a senior living environment.

Key Responsibilities:

• Administer medications and treatments following physician orders and the Six Rights of Medication Administration
• Accurately document in MAR/TAR in real-time
• Monitor residents for symptoms, adverse reactions, and changes in physical, emotional, or cognitive status
• Support care plan updates by transcribing physician orders and conducting Service & Health Updates (SHUs)
• Report any medication refusals, adverse reactions, or health concerns promptly
• Maintain medication carts, perform narcotic counts, and ensure regulatory compliance
  
  

Qualifications & Skills:

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment, observation, and documentation skills
• Experience in senior care or medication administration preferred
