What are the responsibilities and job description for the Client Care Coordinator position at Live-Ins for the Elderly?
We are looking for a Client Care Coordinator for our Home Care Agency in Danbury, CT.
The coordinator must have a minimum of 2 years experience in this field or comparable.
The office hours are Monday to Friday from 9am-5pm, with on call duties.
The purpose of the client care coordinator is:
To facilitate a good match between the caregiver and the client to promote home care as a viable and desirable alternative to institutional care.
Responsibilities and Duties
The coordinator must
- Plan, Coordinate, and implement all homecare services, including client monthly visit schedules and follow-up status checks after services implemented, and client hospitalizations.
- Conduct in home client assessments, periodical home visits and state mandatory 60 days reports.
- Assist with administrative and general office work
- Conduct Caregiver's job interview to ensure qualification and placement.
- Utilize a “common sense” approach with client situations, giving paramount importance to the needs of the client.
- Perform quality assurance type calls to clients to check how services are going, make necessary updates to client/caregiver information and fill out any necessary forms/notes to document these calls.
- Schedule the best possible caregiver to promote a long-term relationship between the caregiver and the client.
- Fill last minute call outs, and short-notice new cases.
- Answer phone inquiries regarding the services we provide and the positions we have available.
- Escalate any concerns with meeting a client’s needs or with creating a consistent schedule for a client to your supervisor.
- Monitoring day-of schedules and troubleshooting problems, and responding to client or caregiver issues as they arise.
- Participate in on-call duties.
- Maintain accurate schedules, clock-in/clock-out timecards, and clear notes for exceptions in the Company's systems, as required, to ensure accurate billing and payroll function.
Qualifications and Skills
Compassion for the elderly, ability to multi-task, basic computer skills, straight forward communication skills, common sense, polite phone manner and a dedication to the company's mission.
- Good telephone, organizational, multitasking, typing and communication skills necessary.
- EVV and Generations knowledge is a plus.
- Proficient in Microsoft Office Suite, Home Care Scheduling Platforms, Internet.
- Bi-lingual, any other language is a plus
- 2 Years work related experience within the health care services industry preferably home care industry.
