Job Description
Job Summary
The Social Worker is responsible for ensuring that the social and emotional needs of residents are met while coordinating safe, successful discharges.
Key Responsibilities
Resident Assessments & Care Planning
- Conduct initial interviews and psychosocial assessments, including social history, adjustment level, support systems, and interests.
- Complete assigned MDS sections and develop individualized care plans within required timeframes.
- Enter timely, complete, and meaningful progress notes into the medical record.
- Develop and implement comprehensive discharge plans that address all resident needs.
- Coordinate initial care conferences within 10 days of admission.
- Collaborate with physicians, nursing, rehab, residents, and families to ensure seamless transitions.
- Ensure discharge services and equipment are in place, aiming to prevent hospital readmission within 90 days.
- Receive and resolve concerns or grievances promptly; maintain a grievance log.
- Facilitate communication between residents, families, and staff to ensure positive relationships.
- Provide information regarding financial assistance and required administrative forms.
- Help residents and families access community resources and support services.
- Administer NOMNC and ABN notices per Medicare and managed care requirements.
- Participate in the facility’s QAPI program and present monthly improvement projects.
- Assist in locating lost items and distribute required letters or notices as needed.
- Maintain strict HIPAA compliance and prepare clear, concise written reports.
- Represent the facility professionally in interactions with outside organizations.
- Bachelor’s degree in Social Work or related field (required).
- Certified or licensed by the NJ State Board of Social Work Examiners.
- Experience working with older adults strongly preferred.
- Ability to multitask, prioritize, and meet deadlines with minimal supervision.
- Strong communication skills and ability to work effectively with physicians, rehab, nursing, and administration.
- Proficient in technology, including:
- Microsoft Office (Excel, Word, Outlook required)
- Teleconferencing platforms
- Electronic medical records—PointClickCare (PCC) experience strongly preferred
- Complies with all Lions Gate policies, including ethical business practices and Corporate Compliance.
- Maintains current knowledge of State and Federal regulations, OSHA standards, and mandatory education.
- Ensures confidentiality of resident and facility information at all times.
- Follows resident rights, abuse prevention, incident reporting, safety, and infection control procedures.
- Adheres to Kosher dietary laws in designated areas.
