What You'll Do
- Advocate for resident rights, preferences, and overall well-being
- Communicate with families (with resident consent) regarding care updates and discharge planning
- Coordinate referrals and services such as community resources, financial assistance, and supportive programs
- Assist residents with financial and legal processes (Social Security, Medicare, Medicaid, VA benefits, etc.)
- Build strong relationships with residents and collaborate with staff to support individualized care
- Participate in interdisciplinary care plan meetings and help develop effective care strategies
- Address and help resolve resident or family concerns and grievances
- Complete social service assessments and discharge planning documentation
- Maintain accurate, timely records in the electronic medical system
- Provide guidance and oversight to social services team members as needed
- Experience in a long-term care setting is preferred but not required
- Experience with electronic medical records (EMR)
- Strong communication, problem-solving, and organizational skills
- Ability to manage sensitive situations with professionalism and empathy

- Bachelor's degree in Social Work or Human Services required; Master's degree preferred
- LCSW strongly preferred
- Minimum of one year of supervised social work experience in a healthcare setting
