About the Role:
The Care Coordinator plays a pivotal role in ensuring that patients receive comprehensive, timely, and effective healthcare services by acting as a liaison between patients, healthcare providers, and community resources. This position is responsible for developing, implementing, and monitoring individualized care plans that address the unique needs of each patient, promoting continuity of care and improving health outcomes. Minimum Qualifications:
- Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or a related field.

- Proficiency with electronic health record (EHR) systems and standard office software.
- Excellent communication and interpersonal skills to effectively interact with diverse patient populations and healthcare teams.
Preferred Qualifications:
- Certification in Case Management (CCM) or Certified Care Coordinator credentials.
- Experience working with chronic disease management or behavioral health populations.
- Familiarity with insurance processes, including Medicaid and Medicare.
- Bilingual abilities, particularly in Spanish or other commonly spoken languages in the United States.
- Advanced training in motivational interviewing or patient education techniques.
Responsibilities:
- Assess patient needs and develop personalized care plans in collaboration with healthcare providers and patients.
- Coordinate and schedule medical appointments, treatments, and follow-up care to ensure seamless service delivery.
- Serve as the primary point of contact for patients, families, and healthcare teams to facilitate effective communication.
