Community Participation Professional
Hours: Monday – Friday 7:30am – 4:00 pm/3:30pm if working lunch
$500 sign on Bonus and general PTO package
The Community Participation Professional will be responsible for supporting adults with intellectual and developmental disabilities (IDD) to engage in meaningful, inclusive, and enriching community activities. This role involves planning, coordinating, and facilitating opportunities for individuals to participate in a variety of vocational, educational social, and recreational experiences that enhance independence, self-determination, and integration into the community.

Qualifications
- Associate degree in Human Services field Or high school diploma/GED with one year of experience in human services
- Current PA driver’s license with clean driving record
- Pass pre-employment physical and PA State Police Background Check
- Pass a Lighthouse Vocational Services driver’s test
- Knowledge of IDD and strategies to support community inclusion
- Strong communication and interpersonal skills, with the ability to work well with individuals, families, and teams
- Ability to multitask
- Experience working with adults with intellectual and developmental disabilities (IDD) preferred.
- Ability to work independently and as part of a team
- Patience, empathy, and a commitment to promoting the rights and well-being of individuals with IDD
- Basic computer skills for daily documentation/email
- Complete CPS training within thirty days of hire
- Works with individuals with IDD to create personalized community participation plans, based on their interests, goals, and abilities.
- Reads and signs ISP and SEEN plans for each individual, and learns the essential information for each; medications, physical limitations, allergies, behavioral concerns, adaptive equipment needs, and any safety precautions needed, knowing each level of supervision needed while in the community
- Leads or supports participation in volunteer, educational, recreational, and social, activities within the local community, ensuring they are safe, engaging, and promote skill-building.
- Acts as an advocate for individuals with IDD by promoting their rights, ensuring they have equal access to community resources, and helping them navigate community-based systems and services.
- Teaches and supports skills such as communication, socialization, job readiness, self-care, and safety while interacting in various community settings.
- Regularly communicates with the Supervisor to assess progress, review goals, and adjust community participation plans as needed.
- Tracks participation, skill development, and outcomes for individuals. Document and maintain detailed records of activities, progress reports, and incident reports in Setworks
- Ensures that all activities are conducted safely and in compliance with relevant regulations, including safeguarding the health and well-being of the individuals under care.
- Assists participants with personal care needs if necessary (including toileting)
- Crisis management skills and the ability to maintain composure in challenging situations
- Possesses strong organizational and time-management skills
- Possesses knowledge of local community resources and services
- Advocates for the rights of individuals with IDD in various settings, ensuring compliance with applicable laws and regulations.
- Works as a team player with colleagues to promote a positive and engaging work environment
- Participates in ongoing professional development and attend relevant training to stay current with best practices in the field.
- Completes annual required online training on Relias training platform
- Attends professional development/team building training in person

- Completes other tasks as assigned
