Summary
To provide direct care support and opportunities for individuals through supervision, role modeling and teaching. To help develop and maintain work and life skills and assist individuals to establish a network of relationships in the community that enrich their lives. Complies with all principles of Human Rights, D.P.P.C., Department of Developmental Services, the Division of Medical Assistance and all other applicable funding regulations as adhered to by Lifeworks.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Promotes optimal health for individuals though routine, preventive, and emergency health care. Ensures that work environments and Life Enhancement activities are safe, secure and where individuals are free from harm.
  • Provides supervision, training and coaching across all settings including Life Enhancement Program activities, that will increase the individual's self-sufficiency and independence, and to meet the needs and expectations of employers, volunteer sites, families and individuals supported. Will be able to provide support to employers as necessary and will ensure quality and consistency of work performed by these individuals.
  • Provide education training, counseling, guidance and physical assistance when necessary to achieve person's individual goals in the following areas: employment, education, social growth, health and wellness, communication skills, community mobility and volunteerism.
  • Develop, write and present annual Individual Support Plans (ISP's) and the subsequent support paperwork in consultation with their supervisor. Responsible for advocating and ensuring that ISP plans are implemented to achieve the individual's goals through employment and the Life Enhancement Program.
  • Performs case management duties including documentation of training and activities, maintenance of individual confidential files on assigned caseload, review and submit all required paperwork to supervisor according to agency procedures. Attends regular meetings as necessary.
  • Ability to be flexible regarding necessary changes in work schedule and tasks as assigned by supervisor to cover the needs of the individual supported.
  • Maintains regular communication with individuals and all ISP team members and other persons significant to the person served (i.e. family, guardians, advocates) to facilitate consistency of programming and support. Acts as liaison between Lifeworks, employers, volunteer sites and community-supported sites in order to ensure that individuals are meeting all expectations in the community. Maintains confidentiality and professional demeanor with regards to all individuals and staff members.
  • Provides transportation to and from various sites in the community by either Lifeworks or personal vehicles as necessary. Staff are reimbursed at the agency mileage rate for any use of their personal vehicle within the guidelines established by Lifeworks.
  • Ability to demonstrate good computer skills as well ability to learn to utilize the agency's computer programs for documentation of necessary data.
  • Other duties and/or assignments to work/program locations based on the needs of the company.

Supervisory Responsibilities
This job has no supervisory responsibilities of other staff.

EDUCATION And/or EXPERIENCE
BA/BS in Human Services or related plus 3 years related experience with persons with ID/DD or Human Services

Language Skills
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write and proofread routine reports and correspondence. Ability to speak and present effectively before individuals or employees of an organization.

REASONING ABILITY
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

VEHICLE/TRAVEL
Requires a legally insured and inspected automobile for use to be presented at work and available for use during working hours. If the program has an agency vehicle, staff are expected to drive it as needed.

CERTIFICATES, LICENSES, REGISTRATIONS
Valid and current driver's license from the state of residency, Medication Administration, CPR, First Aid certification.

While performing the duties of this job, the employee is frequently exposed to outside weather conditions. The employee is occasionally exposed to wet and/or humid conditions, moving mechanical parts, and vibration. The noise level in the work environment is usually moderate.

During the performance of the duties of this job, the employee can be expected to have meal breaks with the individuals served and to provide them with any assistance that they need during these meal breaks.
