Care Coordination, Discharge Planning, Utilization Review
Minimum Required Qualifications
2 Years case management experience
This position does not require an RN, it could be a case manager with another clinical background such as social worker.
2 Years Of Rehabilitation Experience Preferred.
LENGTH OF ASSIGNMENT
13 Weeks
SHIFT / HOURS PER WEEK
Days, M-F 8a-5p (No Weekends)
