What are the responsibilities and job description for the Nurse Care Manager position at LifeWays?
The Nurse Care Manager provides culturally competent, trauma-informed care to consumers through assessment, care coordination, advocacy, and person-centered planning. Under the direction of the Supervisor of Integrated Health Services, this role coordinates medical, behavioral health, and community services to ensure timely, appropriate, and goal-oriented care. The position supports the LifeWays Substance Use Disorder Health Home (SUDHH) model and upholds LifeWays’ mission, vision, and values.
Key Responsibilities

Engage consumers with empathy, professionalism, and a non-judgmental approach

Conduct assessments, reassessments, and person-centered service planning

Coordinate referrals and care within and outside the provider network, including Outpatient Psychiatry

Collaborate with psychiatrists, primary care providers, specialists, and community partners

Monitor caseloads and ensure ongoing medical necessity and appropriate levels of care

Support transitions across levels of care and life stages

Assist with crisis and after-hours planning

Provide services in person, by telehealth, and by phone

Perform care management functions for the SUDHH program
- Nursing Care Manager I: Associate degree in nursing. Licensed as RN.
- Nursing Care Manager II: Bachelor’s degree in nursing. Licensed as RN.
- Master’s degree in human services related field.
- Nursing Case Management Certification (CMGT-BC)
