- Support daily operations of Day Habilitation Services and assist in delivering person-centered programming.
- Assist in supervising, training, scheduling, and supporting Day Habilitation staff.
- Help develop, implement, and monitor individualized Day Hab plans and outcomes.
- Coordinate community volunteer opportunities and meaningful integrated activities for individuals supported.
- Collaborate with families, interdisciplinary teams, and community providers to ensure coordinated services.
- Monitor service delivery at home base and community sites and coordinate transportation supports.
- Complete and maintain required documentation, reports, and compliance records.
- Promote a professional, inclusive, and person-centered environment for staff and individuals supported.
- Assist in obtaining equipment and resources necessary to support effective services.
- Bachelor’s degree in a related field with one (1) year of experience supporting individuals with disabilities; OR
- Associate degree with three (3) years of related experience; OR
- High school diploma/equivalent with five (5) years of related experience, including supervisory experience.
- Strong communication, leadership, and organizational skills.
- Ability to interpret policies, service plans, and regulatory requirements.
- Demonstrated problem-solving and decision-making abilities.
- Must meet Lifetime Assistance Vehicle Operator Requirements.
- Ability to meet physical requirements of the role, including lifting up to 50 pounds and maintaining SCIP-R certification if required.
