POSITION SUMMARY:
Under direct supervision of the PASRR Manager, the Regional Case Manager is responsible for providing case management services to individuals in PASRR/SGF or Traditional Wavier programs.
DUTIES AND RESPONSIBILITIES:
ESSENTIAL FUNCTIONS:
- Responsible for assuring that individuals rights are respected and individuals are free of any and all forms of abuse or mal treatment.
- Responsible for supporting individuals to maximize personal choice and honoring those choices within reason.
- Responsible for developing person centered plans based on individual preferences and needs using a person-centered approaches.
- Responsible for completing one face-to-face contact per month with each client on assigned caseload.
- Responsible for coordinating individual supports as necessary and available, with input from team members.
- Responsible for proper and appropriate documentation of all supports provided in a timely manner.
- Responsible for having a working knowledge of DDID/SGF/ MPW wavier regulations and maintaining appropriate documentation to meet these guidelines.
- Responsible for completing billings for appropriate supports provided in a timely manner.
- Responsible for documenting all incidents and accidents in a timely manner, on the appropriate form, and notifying the appropriate parties.
- Responsible for monitoring and completing all required forms, via RedCap or MWMA, for the purpose of wait-listing individuals on assigned caseload.
- Provide crisis intervention, direct support, and transportation to clients as needed
- Responsible for attending all necessary training and updates as needed and Perform Level II Evaluations as needed.
- Complete MAP 351 Assessments as needed.
- Perform other duties as assigned by supervisor.
JOB REQUIREMENTS / EXPECTATIONS:
Case manager would need the ability to lift up to 15lbs PRN, must be able to have close, distance, peripheral and depth vision with the ability to adjust focus. Individual must have the ability to stand walk, climb, balance, stop, kneel, crouch or crawl one third of the time. Also, must being able to use fine motor skills, the ability to speak, hear and reach more than two thirds of the time.
The individual workplace environment includes community settings, Nursing Facilities, hospitals and in home visits with travel up to 50% of the time.
The case manager role will be evaluated annual by performance review. Case Manager duties and responsibilities will be reviewed monthly through case managements or more often as needed to maintain Federal and State requirements.
- Face to face monthly visits (home/ nursing facility/ day program)
- Maintaining monthly contact notes and summaries in Carelogic/ MWMA in timely manner.
- Maintaining Annual PCSP (plan of care) and other supporting documents in a timely manner.
- Completing all incident reports and follow ups in RedCap and MWMA in a timely manner.
- Participate in team meeting, trainings and other assigned events as needed.
- Perform and complete PASRR Level II Evaluation and MAP 351 Assessments as needed.

- Bachelor’s Degree in Related Field
- Minimum one year experience working with individuals with Developmental/ Intellectual Disabilities.
- Valid drivers’ license and proof of insurance.
- Knowledge of community resources and ability to develop/create appropriate support resources.
