Job Summary
The Senior Choice Transportation Coordinator is responsible for daily scheduling, communicating with clients and staff, maintaining an up-to-date transportation database, and ensuring timely documentation and follow-up. The Senior Choice Transportation Coordinator is responsible for daily scheduling, communicating with clients and staff, maintaining an up-to-date transportation database, and ensuring timely documentation and follow-up. Essential Responsibilities
- Lead Transportation Coordinator, responsible for scheduling, communications with clients, documentation and follow up.
- Back up Transportation Driver as needed
- Coordinate Transportation trips with local community sites and Senior buildings
- Accurately complete and process referrals for clients to other agencies and private individuals.
- Provide accurate Information and Assistance for walk-in or telephone consumers.
- Stay up to date and informed on available services and how to access those services for consumers.
- Accurately and timely completion of various forms and applications for consumers with information provided by the consumer.
- Perform home visits for the purpose of assessing consumer needs, identifying available assistance and completing necessary forms, phone calls, etc. to assist the consumer in accessing needed services.
- Inform and assist clients in gaining access to available benefits and services including acting as representative at appropriate agencies with consumer consent.
- Provide timely, accurate and ongoing follow-up with consumers to insure their needs continue to be met.
- Maintain timely and accurate required documentation of service given to clients.
- Must be able to effectively and patiently communicate with consumers, caregivers, staff and visitors.
- Must be able to interpret consumers or caregivers needs effectively.
- Must be able to work independently.
- Must be able to obtain a jo5.
- Associate Degree preferred minimum of 3 years of customer service and data entry experience. Must have 1 year experience working with the senior population. Ability to work independently and collaboratively in team and demonstrate commitment to cultural diversity.
