Reports To: Program Services Director
Salary: $65,214.00
Position Type: Full-Time Monday-Friday

Position Summary
Under the direct supervision of the Program Services Director, the Care Coordination Unit (CCU) Supervisor is responsible for the overall day-to-day management of the Care Coordination Unit in accordance with agency policies, funding requirements, and applicable laws. This position provides leadership and supervision to CCU staff, ensuring effective service delivery, compliance, and quality outcomes. The Supervisor also manages the on-call schedule and participates in the on-call rotation as needed. Key duties include interviewing, onboarding, managing and training staff; assigning and directing work; appraising performance; networking and marketing the program; ensuring quality assurance and compliance; rewarding and disciplining employees; and addressing and resolving complaints or problems. Additional responsibilities include ongoing program monitoring and other duties essential to program operations.

Qualifications
Education and Certification
- Bachelor's degree in Social Work, Human Services, Healthcare, or a related field required; Master's degree preferred.
- Relevant professional certifications (e.g., CCM, LCSW, RN, or equivalent) preferred.
- Must maintain any required state or program-specific certifications.
- Minimum of three years' supervisory or management experience in healthcare, human services, or social services programs, preferably with older adults or care coordination.
- At least two years' experience providing direct client services in a related field.
- Experience with program compliance, quality assurance, and reporting to funding or regulatory agencies.
- Experience in community outreach, partnership development, and staff training is highly desirable.
- Strong leadership and team management skills, including the ability to motivate and develop staff.
- Excellent communication, interpersonal, and conflict resolution abilities.
- Ability to manage multiple priorities, adapt to changing needs, and maintain attention to detail.
- Proficiency in electronic health records (EHR), case management systems, and standard office software.
- Knowledge of relevant laws, regulations, and best practices in care coordination and human services.
- Ability to represent the agency professionally in all interactions with clients, staff, and community partners.
- Must be available to participate in the on-call rotation and respond as needed.
- Must pass applicable background/drug screening checks and meet agency eligibility requirements.
