Day Hab Direct Support Professional
FULL TIME & PART TIME
Contracted position
Responsibilities include:
- Provide direct support care for individuals with developmental disabilities.
- Implement the individual’s personal support plan.
- Provide supervision and guidance for the supported individual(s) at day center and in the community.
- Assist with day center activities and community outings.
- Administer, document, and monitor the administration of individual(s) prescribed medications.
- Communicate ongoing care results and updates to relevant parties.
- Complete daily logs and reports as outlined by agency policy..
- Develop a rapport with residents and cultivate a safe and supportive relationship.
