Description
The higher level of services through TCOOMMI is intended to stabilize the individual’s mental health symptoms, reduce the risk of recidivism, increase awareness of and participation in community and natural supports, develop client skills in self-advocacy, extend client tenure in the community, and assist clients in gaining the ability to participate in independent mental health care. Ensures services provided are in accordance with the TCOOMMI Program Guidelines and processes for ICM. Provides directed services to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services. Provides person directed skills training and psychosocial rehab to develop coping and other skills needed to feel successful in their home and community. Requires use of initiative and independent judgment to provide interventions in the community, home and school to individuals on caseload. Is responsible for initiating and collaboratively completing uniform assessments then organizing and documenting the plan for services to meet the individual’s identified needs. Must be effective at troubleshooting and organizing support, interventions, and services to achieve outcomes. Must have the ability to work independently and collaboratively as needed and have effective communication with supervisory staff when necessary. Expectations include timely and thorough documentation, time management and organizational skills, effective scheduling, ability to efficiently manage clinic flow, adhere to crisis procedures and be available to assist with crisis and other coverage areas as needed.
Provides community-based person directed case management to assist in navigating access to medical, social, vocational, financial, educational, legal, and other individualized services. Is responsible for initiating and collaboratively completing uniform assessments, then organizing and documenting the plan for services to meet the individual’s identified needs in the community, home, legal system, and school. Must be effective at troubleshooting and organizing support, interventions, and services to achieve outcomes. Position will work closely with criminal justice agencies. The goal is to link criminal justice involved individuals with appropriate mental health and substance use disorder services to reduce recidivism and restore competency while treating serious mental health concerns to ensure individuals receive the services that they need to remain stable, function in the community, and regain competency when possible. Other duties as assigned to meet program and individual needs.
This position practices good stewardship of resources by ensuring all required background checks and training are completed prior to due date and center-issued equipment is maintained in good condition. The TCOOMMI TJJD LPHA will link individuals to additional resources as needed to support their goals.
Integrity is necessary to ensure adherence to the Health and Human Services Commission, Texas Administrative Code, contracts, applicable grants, and Lifepath Systems policies and procedures. This position will work closely with fidelity managers and leadership to ensure that all gaps are reconciled.
This position builds positive relationships with local court and jail staff, the juvenile justice probation office, and mental health professionals in an effort to increase referrals from community stakeholders and partners. This position demonstrates professional conduct with Peers, Leadership and Support teams at all times. In addition, close working relationships and professional conduct while coordinating care with outside entities is required.
Working independently and collaboratively to maintain appropriate boundaries and foster better outcomes for individuals served and facilitating improvement in measurable ways, is central to LifePath Systems mission to serve.
Specific Targets: Engagement with individuals served and exceptional customer service will be provided and documented in the electronic health record. At least 90% of service notes are signed within 2 business days of service. All full chart reviews receive a score of 80% or higher following the Golden Thread. Provides the first service, completing the intake, CANS, TRR, and RP within 3 working days of receiving the referral from TCOOMMI. Ensures individual length of stay does not exceed 24 months. Updates individualized treatment plans within five business days of enrollment into the program. Maintains data collection and reporting standards.
If Bilingual and receiving stipend, employee will be available to translate for other staff as needed and will utilize approved language in the provision of services as needed.

Qualifications
- Master’s degree in counseling, social work, or related field required.
- Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) or Licensed Marriage & Family Therapist (LMFT) required.
- CCBHC knowledge and experience preferred.
- CBT, CPT, TF-CBT, Play Therapy, and/or PCIT certification preferred.
- Knowledge of Trauma-Informed Care Practices required.
- At least two years of related experience and/or training in the field of mental health with jail diversion or juvenile justice preferred.
- Previous work experience, partnership, or collaboration with court and justice systems highly desired.
- Must have reliable transportation and a valid driver’s license.
- Knowledge of the current version of the Diagnostic and Statistical Manual of Mental Disorders (DSM).
- Knowledge and experience in providing jail diversion services, psychotherapy, case management, skills training, and symptom management services to individuals with mental illness.
- Ability to assess individual needs, to coordinate services, and to communicate effectively.
- Experience providing psychosocial assessments, treatment planning, service coordination, skills training, counseling, and crisis intervention services to individuals with serious mental illness or serious emotional disturbance.
- Knowledge of the social services system and the ability to connect individuals with appropriate services.
- Ability to work independently and collaboratively within a team, taking initiative and utilizing judgment appropriately.
- Timely and thorough documentation, time management and organizational skills, effective scheduling, ability to efficiently manage workflow.
- Ability to respond to individuals with patience, empathy, and understanding.
