The position is part of the Self-Direction Program, and we work with many different FI agencies. Responsibilities
- Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support, using equipment such as Hoyer lifts when necessary.
- Prepare nutritious meals in accordance with dietary guidelines and assist residents during mealtimes to promote independence.
- Maintain a clean, safe environment by performing routine cleaning tasks and adhering to infection control protocols.
- Document care activities accurately in compliance with privacy laws and organizational policies, including progress notes and incident reports
- Encourage social interaction and engagement in community activities and support participation in preferred activities such as music, movies, pet interaction, and simple cooking activities (with hand-over-hand assistance as needed)
- Maintain close supervision at all times to ensure safety as required.

Skills
- Proven experience working in group homes, assisted living, nursing homes, or home health environments with individuals who have developmental disabilities or aging-related conditions is a plus
- Familiarity with emergency procedures such as CPR and first aid certification to respond effectively to crises is a plus.
- Strong communication skills for collaborating with healthcare teams, residents, families, and community resources
- Ability to perform physical tasks including heavy lifting and assisting with mobility aids like Hoyer lifts or member transfers a plus.
- Understanding of HIPAA regulations to protect resident privacy and confidentiality
Requirements
· High school diploma or GED, or the ability to pass an Educational Proficiency Testing
· Valid NYS driver’s license (If you do not have one, please note that as some agencies do not require one)
· Reliable transportation (you do not need always need your own car)
· Ability to provide close supervision and follow detailed support plans
· Comfort working around medical equipment (training provided, no clinical responsibility)
· Ability to remain calm, attentive, and patient in a medically/behavior involved environment if needed
· Willingness to complete required OPWDD self-direction onboarding and documentation
· Ability to work independently while respecting family leadership and boundaries
- Smartphone and/or tablet for electronic record keeping.
