Registered Nurse (RN)

You will be responsible for coordinating and managing the care of patients in their homes as a Home Health RN Case Manager. Registered nurses in this role need to have strong clinical skills, to be able to work independently, and to provide patient-centered, high-quality care.

Key Responsibilities:

Assessment and Care Planning:

• Conduct comprehensive assessments and reassessments of patients' health needs in their home environments.
• Develop individualized care plans that address the unique requirements of patients receiving care at home.
• Coordinate with interdisciplinary teams, including home health aides, LPNs, and therapists to ensure a holistic approach to patient care.
• Schedule and organize home visits, ensuring timely and effective delivery of healthcare services.

Patient and Family Education:

• Educate patients and their Health Aids on managing health conditions, medications, and any required lifestyle modifications.
• Provide guidance on recognizing and responding to changes in the patient's condition.

Direct Patient Care:

• Provide hands on care to patients, including but not limited to wound care, IV infusions, blood draws, central line maintenance, catheter care, ostomy care and education, etc.

Monitoring and Evaluation:

• Regularly assess patient progress and modify care plans as needed.
• Monitor vital signs, medication adherence, and overall well-being during home visits.

Documentation and Reporting:

• Maintain accurate and up-to-date electronic health records.
• Prepare and submit reports documenting patient progress, changes in condition, and any relevant information for interdisciplinary team meetings.

Communication:

• Collaborate with physicians and other healthcare professionals to coordinate care and address patient needs.
• Communicate effectively with patients, families, and caregivers, ensuring clear and concise information exchange.

Quality Assurance:

• Adhere to quality assurance and compliance standards for home health services.
• Participate in quality improvement initiatives to enhance the overall quality of patient care.

Qualifications:

• Registered Nurse (RN) license in good standing.
• Bachelor of Science in Nursing (BSN) preferred.
• Previous experience in home health nursing or a related field.
• Strong clinical assessment and critical thinking skills.
• Knowledge of home health regulations, reimbursement systems, and documentation requirements.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of an interdisciplinary team.

Pay: $30.00 - $40.00 per hour

Schedule:

• Few hours a week (would be agreed upon hire)
• Weekends as needed
