You will be responsible for coordinating and managing the care of patients in their homes as a Home Health RN Case Manager. Registered nurses in this role need to have strong clinical skills, to be able to work independently, and to provide patient-centered, high-quality care.
Key Responsibilities:
Assessment and Care Planning:
- Conduct comprehensive assessments and reassessments of patients' health needs in their home environments.
- Develop individualized care plans that address the unique requirements of patients receiving care at home.
- Coordinate with interdisciplinary teams, including home health aides, LPNs, and therapists to ensure a holistic approach to patient care.
- Schedule and organize home visits, ensuring timely and effective delivery of healthcare services.
Patient and Family Education:
- Educate patients and their Health Aids on managing health conditions, medications, and any required lifestyle modifications.
- Provide guidance on recognizing and responding to changes in the patient's condition.
Direct Patient Care:
- Provide hands on care to patients, including but not limited to wound care, IV infusions, blood draws, central line maintenance, catheter care, ostomy care and education, etc.
Monitoring and Evaluation:
- Regularly assess patient progress and modify care plans as needed.
- Monitor vital signs, medication adherence, and overall well-being during home visits.
Documentation and Reporting:
- Maintain accurate and up-to-date electronic health records.
- Prepare and submit reports documenting patient progress, changes in condition, and any relevant information for interdisciplinary team meetings.
Communication:
- Collaborate with physicians and other healthcare professionals to coordinate care and address patient needs.
- Communicate effectively with patients, families, and caregivers, ensuring clear and concise information exchange.
Quality Assurance:
- Adhere to quality assurance and compliance standards for home health services.
- Participate in quality improvement initiatives to enhance the overall quality of patient care.
Qualifications:
- Registered Nurse (RN) license in good standing.
- Bachelor of Science in Nursing (BSN) preferred.
- Previous experience in home health nursing or a related field.
- Strong clinical assessment and critical thinking skills.
- Knowledge of home health regulations, reimbursement systems, and documentation requirements.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of an interdisciplinary team.
