Job Purpose:
- This position is responsible for supporting the intake, screening, and enrollment process for home-delivered meals and nutrition programs at LIFECARE ALLIANCE. The Intake Specialist plays a crucial role in ensuring that clients receive the necessary services by efficiently managing the initial stages of client engagement and program eligibility.
Key Responsibilities:
- Intake Processing & Client Engagement: Conduct initial client assessments, gather necessary information, and engage with clients to ensure a smooth intake process.
- Eligibility Determination & Communication: Evaluate client eligibility for programs and effectively communicate determinations to clients and relevant stakeholders.
- Systems, Compliance & Data Entry: Accurately enter client information into management systems, ensuring compliance with organizational policies and procedures.
- Coordination & Internal Collaboration: Work collaboratively with internal teams to coordinate client services and ensure a seamless experience for clients.

Required Education:
- High school diploma or equivalent
Preferred Education:
- Preferred completion of an associate’s degree in human services, Business Administration, or a related field
Required Experience:

Preferred Experience:
- Preferred familiarity with Medicaid, Older Americans Act programs, or other publicly funded services
Required Skills and Abilities:
