JOB SUMMARY:
In collaboration with the appropriate multidisciplinary personnel, the Heart failure navigator (HFN) coordinates transition of care for the heart failure patient from hospital discharge to home or skilled nursing facility to achieve desired patient and program outcomes. The HFN ensures that all aspects of follow-up care for the heart failure patient are addressed and met. Additionally, the HFN provided education as appropriate. The HFN works in partnership with other staff members and proactively identifies and provides for the needs of patients.
- Responsible for identifying and meeting with the patient to educate about heart failure and implications for follow up care @ Sinai and Northwest hospitals.
- Teaches heart failure patients self-management strategies with evidence based practice.
- Works as a collaborative member of the clinical team to ensure quality services and advocates for the patient.
REQUIREMENTS:
- Basic professional knowledge; equivalent to a Bachelor's degree; working knowledge of theory and practice within a specialized field.
- RN license in the State of Maryland (or Compact licensure as applicable)
- 3 years of experience in Cardiology required; 5 years of experience preferred
- Current BLS Provide Certification from the American Heart Association required.
