What are the responsibilities and job description for the Community Health Worker position at LifeBridge Health?
Summary
The Community Health Worker provides direct services by means of education, advocacy and services coordination to individuals in order to extend psychosocial support services.
Responsibilities include:
Gathers information related to individual and family, financial, health, and emotional status in order to develop plan for addressing psychosocial barriers to health and well-being of clients to develop a client directed Service Plan outlining services. Completes a needs assessment through face-to-face meetings with clients during home, community and/or office visits.
Provides education and psychosocial support on issues such as health, medical compliance, budgeting, safety, parenting and community resources according to assessed need and resulting plan. Identifies educational and support needs through the needs assessment.
Maintains information on available community resources, makes appropriate and timely referrals for services, and provides ongoing coordination of needed services. Assists program participants with negotiation of complex systems such as human services, health, legal, etc. Refers participants to community resources that meet assessed needs.
REQUIREMENTS:
- Formal working knowledge; equivalent to an Associate's degree (2 year's college); requires knowledge of a specialized field. Preferred: * Associate's Degree
- 1-3 years of experience.
- Driver's license and personal transportation is preferred and may be required for some service areas.
