Nurse Practitioner/Physician Assistant

Position Overview We’re seeking a compassionate and skilled Nurse Practitioner (NP) to provide clinical services directly to patients at their place of residence. The ideal candidate is adaptable, independent, and committed to delivering patient-centered care in a variety of settings.
Employment Type
- Full-time, Per-diem, Part-time or Full time, hourly position
- $80/hour
Responsibilities
- Conduct patient assessments, develop treatment plans, and provide ongoing management
- Perform physical exams, order and interpret diagnostic tests
- Collaborate with physicians, families, and facility staff
- Document patient encounters accurately in EMR
- Ensure continuity of care and medication management

Qualifications
Job Qualifications/Skills:
- Current Nurse Practitioner license in New Jersey
- Certification in Adult, Geriatric, or Family Practice preferred
- Minimum 1 year of clinical experience; mobile or home care experience is a plus
- Strong communication and interpersonal skill
- Reliable transportation and willingness to travel locally
- Experience with EMR (electronic medical records),
