Reports To: Director of Client Services
Time Commitment: Full time, 32-40 hours/week, Non-Exempt
Compensation: $15.50-17.50/hr
Job Purpose/General Summary:
The Day Habilitation Supervisor is responsible for the development, implementation, coordination, and provision of program services to participants in the Day Habilitation program under the direct supervision of the Director of Client Services. This is a full-time position.
Essential Duties:
This position includes, but is not limited to, the following essential functions:
- Supervise, direct, and guide clients in the Adult Day Habilitation training program.
- Supervise and provide guidance to clients at programming activities and events.
- Develop and implement daily activities, including daily outings.
- Ensure regulation standards are met for client services
- Ensure client safety at all times
- Market the day habilitation program and recruit new clients to program
- Establish and maintain partnerships with local schools and other providers, including giving presentations, attending program fairs, and other similar activities.
Duties of Position: Programs and Activities.
- Implement, supervise, and encourage client participation in social/recreational activities.
- Implement, supervise, and encourage client participation in physical fitness programs and activities.
- Provide regular life skill development programming for clients
- Develop, supervise, and encourage client participation in community events and activities.
- Develop activities and programs to enhance client’s ability to care for their own health and hygiene.
- Encourage and promote client knowledge and awareness of the activities available in the community.
Duties of Position: Organization and Administration
- Monitor, document, and record client programming.
- Comply with all organizational safety and health policies and procedures.
- Comply with all governmental and organizational rules, regulations, policies, and procedures.
- Attend all meetings, training sessions, and workshops as requested for professional development.
- Ensure timely completion of all forms, reports, and documents as required or requested.
- Hire, train, and manage direct support professionals (day habilitation aides).
Duties of Position: Rehabilitation and Training
- Supervise, direct, and provide guidance to ensure client programming in accordance with client goals.
- Provide the establishment, continuation, or revision of client program goals.
- Document client’s progress or results of training towards goals established by IPP.
- Complete all documentation and reports as requested in timely fashion.
- Perform or assist in completion of evaluations, assessments, or other reports as requested.
- Conduct and document monthly client meetings discussing activity options and opportunities.
Duties of Position: General
- Report all incidents/accidents involving clients, staff, or other personnel within one day and document within 24 hours
- Will maintain current certification in CPR and First Aid.
- Attend all scheduled meetings and training classes as requested or directed.
- Monitor, document and report client behaviors and adverse actions with immediate notification to Director of Client Services.
- Supervise day habilitation aides, ensure quality performance, and manage staff schedule.
Employment Requirements:
Education: Associates degree
Specialized Experience: Training experience or previous work experience with persons with disabilities preferred. Person providing direct support shall within 6 months of hire start date, complete at least 9.5 hours of direct support professional training. Required to complete 4 hours of continuing education in direct support services annually. Certification as Medication Manager or able to obtain training and certification within 6 months of start date.
Background Investigation: Must successfully complete Iowa Department of Criminal Investigation background record check, Iowa DHS Child and Dependent Adult Abuse Central Registry background check, and OIG Medicaid Exclusion Registry check.
Other certifications, licenses, or skills: Ability to read, write and understand the English language sufficient to communicate with consumer, staff, and public. Ability to compare, compile, analyze, coordinate, and synthesize data and information. Must be certified in CPR and First Aid or obtain certification as required. Must possess valid state issued Driver’s license with Limited Chauffeurs Class “D” endorsement or obtain within 30 days of hire and meet insurance company’s standard of insurability.
Relations to others: Ability to supervise, negotiate, communicate, and instruct consumers and subordinates. Must possess strong interpersonal skills. Must be able to act independently and make decisions based on Life Skills Training Center’s values and mission.
Physical Demands (Strength): Heavy – exert up to 50 lbs of force occasionally, and/or up to 30 lbs of force frequently, and/or up to 20 lbs of force constantly to lift or move objects and carts.
Physical Demands (Movement): Standing, walking, sitting, stooping, kneeling, reaching, fingering and handling, or with accommodations
Physical Demand (Auditory & Visual). Talking, hearing, and sight sufficient to communicate with consumers and public, or with accommodations. Must be able to tolerate noisy environment or with accommodations.
Environmental Conditions: Ability to tolerate seasonal weather changes. Must be able to work in hot and humid or cold and dry environment. Ability to work in restrictive training environment or with accommodations. Ability to tolerate and participate in activities and events in community and social settings.
Equipment Used: Must possess basic computer and keyboard skills. Equipment to include computers, cell phones, electronic tablets, telephones and other electronic media. Operate vehicles to include cars and vans. Other equipment as may be used at employer locations and as authorized during conduct of training provision to consumer.
