What are the responsibilities and job description for the Community Integration Trainer - Day Program - 7:30am to 3:30pm position at Life Services Alternatives?
JOB SUMMARY:
The prime responsibility of the Trainer is to provide direct supervision and training for each resident enrolled in community integration services. The Trainer reports directly to the Administrator of the Program.
PRIMARY RESPONSIBILITIES:
1. Maintains confidentiality and compliance with HIPAA.
2. Participates as a member of the Interdisciplinary Team. Implements the Individual Program Plan (IPP).
3. Works with residents in activities that provide opportunities for learning and practice of skills tailored to specific needs of the individual.
4. Follows the weekly schedule as planned by the program participants, reports to volunteer sites as scheduled and on time.
5. Ability to interact and develop positive relationships with the volunteer and/or employer representatives while in the community.
6. Communicates with the volunteer coordinator/representative on a regular basis and assist/trains the participants to complete the required task.
7. Models and trains socially accepted behaviors appropriate to the environment.
8. Provide support and training so participants learn to fully participate in activities, and is able to express how current task may relate to future employment opportunities.
9. Documents progress through daily data collection and daily entries in Therap.
10. Keeps the administrator informed of resident progress and provides ideas/recommendation for training objectives
11. Provides assistance and training to each resident during meals and snacks.
12. Administer medications and other physician orders as required
13. Observes and intervenes if an individual’s behavior indicates imminent potential to injure her/himself, others, or property.
14. Responds to emergencies that involve use of medical/behavioral intervention techniques. Initiates CPR, calls 911 for assistance and provides first aid as needed.
15. Attends and participates in training as required.
16. Other duties as assigned and needed.
QUALIFICATIONS:
- Minimum age is 21.
- Holds a current DSP certification and is CPR and first aid certified.
- Displays proficiency in written and oral English.
- Ability to work with minimal supervision and to exercise good judgment in community settings.
- Has a minimum of six (6) months experience in providing direct supervision and services to people with developmental disabilities.
- Demonstrates caring, patience, good customer service skills, and ability to respond to consumers and co-workers in an empathetic manner.
- Maintains sufficient strength, agility, endurance and sensory ability to perform the duties contained in this job description.
EDUCATION:
- Holds a High School Diploma or a GED certificate.
- Attends DDS support professional training Part 1 in the first year and Part 2 in the second year. If no experience, will need 12 continuing education training unites within 6 months of hiring.
ADDITIONAL REQUIREMENTS:
- This position is a mandated reporter, and is required to observe all consumers rights, report unusual occurrences as specified in Title 22, California Code of Regulations.
- Hold a valid California driver’s license, auto insurance and a good driving record.
- Attends Direct Services Professional (DSP) Part 1 within the 12 months of employment, and Part 2 within 24 months.
- Health Screen and TB test clearance not more than 1year prior or 7days after employment.
- Department of Justice (DOJ)/Federal Bureau of Investigation (FBI) fingerprint and criminal record clearance.
- U.S. citizen or alien authorization to work in the U.S. with legal documentation
- Has never been convicted of a crime other than a minor traffic violation (verified by signed statement).
WORKING ENVIRONMENT:
Typical Working Conditions: Primary work location is indoors, outdoors for leisure activities. Works varying hours and/or varying shifts, mandatory overtime may be required to assure the health & safety of our resident. Regular interaction with residents in providing direct services, including group guidance, physical assistance, management of assaultive behavior, and other conditions common to on and off residence training environments.
Risk/Safety Conditions: Will have on-going interaction with residents. Potential exposure to communicable diseases, blood- borne pathogens, medicinal preparations, and other conditions common to a clinical nursing environment. May require some exposure to communicable diseases or bodily fluids.
Essential Physical Activities: To include: walking, sitting, hearing, seeing up close, talking, standing, finger movement, repetitive motions. Pushing and/pulling one wheelchair/gurney with up to 250-pound resident. Is required to lift a maximum of 25 lbs. and will ask for assistance in lifting over 25 lbs. Maintains sufficient strength, agility, endurance and sensory ability to perform the duties contained in this job description.
