Overview
- We’re looking for a compassionate and experienced Nurse Practitioner (NP) with a
- background in Palliative Care to join our mobile medical team. This full‐time position
- involves traveling to Private Homes and Assisted Living Facilities throughout
- Somerset County and nearby areas to support patients with serious illnesses by
- managing chronic and acute conditions, prioritizing comfort, quality of life, and symptom
- relief. The role offers independent practice with flexible scheduling, backed by a
- supportive and collaborative clinical team, while building meaningful relationships with
- patients and their families.
Responsibilities
- Provide comprehensive palliative care to patients with chronic or life-limiting
- illnesses
- Conduct assessments, develop care plans, and manage symptoms
- Collaborate with physicians, caregivers, and facility staff to ensure continuity of
- care
- Educate patients and families on disease progression, treatment options, and
- end-of-life planning
- Document visits and maintain accurate medical records in compliance with
regulatory standards
- Participate in interdisciplinary team meetings and contribute to quality
- improvement initiatives
Qualifications:
- Active Nurse Practitioner license in the state of New Jersey
- Certification in palliative care, oncology or relevant experience strongly preferred
- Minimum of 1 year clinical experience in geriatrics, hospice, or chronic disease
- management
- Strong communication and interpersonal skills
- Ability to work independently and travel to patient locations
