Role Overview
We are seeking a skilled and empathetic Nurse Practitioner (NP) with experience in palliative care to join our mobile medical team. This role is ideal for a clinician who values autonomy, meaningful patient relationships, and interdisciplinary collaboration.
Qualifications:Responsibilities
- Provide comprehensive palliative care to patients with chronic or life-limiting illnesses
- Conduct in-home assessments, develop care plans, and manage symptoms
- Collaborate with physicians, caregivers, and facility staff to ensure continuity of care
- Educate patients and families on disease progression, treatment options, and end-of-life planning
- Document visits and maintain accurate medical records in compliance with regulatory standards
- Participate in interdisciplinary team meetings and contribute to quality improvement initiatives
Qualifications
- Active NP license in the state of New Jersey
- Certification in palliative care or relevant experience strongly preferred
- Minimum of 2 years clinical experience in geriatrics, hospice, or chronic disease management
- Strong communication and interpersonal skills
- Ability to work independently and travel to patient locations
- Reliable transportation and valid driver’s license
