What are the responsibilities and job description for the AFL Provider position at Life Alliance LLC?
Life Alliance is seeking interested parties in the community to allow a gentleman on the NC Innovations waiver to reside in his/her home in an Alternative Family living arrangement (AFL). This individual has an intellectual disability, so applicants must be patient and willing to have a lot of fun! Work will involve providing a safe, stable home, while teaching various activities of daily living, providing transportation into the community and ensuring the general safety and wellbeing of the individual. Assistance with some personal care tasks will also be a part of the daily routine, in addition to recreation and leisure activities. Care will be provided 24 hours a day, seven days a week, with the exception of the times that the individual is out of the home. The AFL Provider will be responsible for taking this individual to all appointments, and for staying home with, or arranging care for, the individual, if the individual is sick and the AFL Provider is unable to care for him.
This opportunity allows for a daily reimbursement rate to be paid to the interested parties acting as room mates and support to the individual. Applicants must be willing to provide the highest quality of care to this individual. No experience required. Must be able to provide high school transcript, North Carolina driver's license, vehicle registration, proof of residency, proof of vehicle insurance, and proof of homeowner's insurance. Driving is a requirement for this position.
