What are the responsibilities and job description for the Shared Living provider position at Liebe's Home of Hope?
Shared Living is delivered in a private home owned or leased by an individual, couple, or a family chosen by the participant, and who is an independent contractor of the provider agency authorized to deliver direct services and support. The Shared Living contractor and the participant live together in the sole residence, and the participant shares daily life with the Shared Living family in their home and community.
Shared Living includes adaptive skill development of activities of daily living (ADLs) such as:
1. Personal grooming and cleanliness
2. Laundry, Bed making and household chores
3. Eating and the preparation of food
4. Inclusive community activities and Transportation
5 . Health maintenance activities
9. Supervision and protective oversight
10. Social and leisure skill development necessary to enable the participant to live in the most integrated setting appropriate to their needs.
Pay: $15.00 - $20.00 per hour
License/Certification:
- Certified Medication Aide (Preferred)
