Position Overview
Unity Developmental Disabilities Care Center, LLC (UDDCC) is seeking compassionate and responsible individuals or families to become Shared Living Providers (SLP).
A Shared Living Provider opens their home to an individual with developmental disabilities and provides daily support in a family-style environment. This is a meaningful opportunity to make a life-changing impact while earning a competitive monthly stipend.
Responsibilities
- Provide a safe, stable, and supportive home environment
- Assist with Activities of Daily Living (ADLs) such as hygiene, meals, and medication support
- Support community integration and social activities
- Transport to appointments and day services as needed
- Maintain required documentation and communicate with the support team
- Implement person-centered support plans
- Ensure participant rights, dignity, and safety at all times
Qualifications
- 21 years of age or older
- High school diploma or GED
- Valid driver’s license and reliable transportation
- Ability to pass background checks (Adult & Child Abuse Registry, Criminal History)
- Willingness to complete required training
- Minimum one (1) year experience working with individuals with developmental disabilities preferred

- HIPAA & Confidentiality
- DD Waiver Overview
